• Adult applicant over 16 years of age who has never had a passport. (DS-11).
• Adult applicant over 16 years of age who has had a passport which was issued more than fifteen (15) years ago. (DS-11).
• Adult applicant over 16 years of age who has had a passport which has been lost or stolen. (DS-11) + (DS-64)
• Adult applicant over 16 years of age who has had a passport which has been damaged.

Passport Renewal (Form DS-82) 

• Adult applicant was over 16 years of age when passport was issued; date of issue
  being within the last Fifteen (15) years. (DS-82).
• Adult applicant with a current passport that was issued more than 1year ago, and needs
  name changed or correction on passport. (DS-82)

• Valid passport needs additional pages added. (DS-4085)

Re-application of passport (Form DS-5504) 

• Valid passport that needs a name change and passport was issued
  less than 1 year ago. (DS-5504)
• Valid passport that needs data correction and passport was issued
  less than 1 year ago. (DS-5504)
• Valid passport that needs to be extended to full validity and passport was issued
  less than 1 year ago.(DS-5504)
